IKA - the Greek national Social Insurance Institution used to send once per year a letter to you (or to your employer) which included the annual sticker for your Ατομικό Βιβλάριο Υγείας Atomiko Bibliario Ygeias (personal health booklet).
From 2015 on, you have to do the renewal in your local Kentro Exypiretisis Politon (KEP - Citizen Service Center), or online by Internet.
The online method is only available for the Greek IKA Booklet. For Health Booklets from abroad, you need to contact your local IKA office and have it renewed there.
For renewing the Greek IKA Booklet, click on https://apps.ika.gr/eInsEligibility/ and fill the following information in the form:
• Αριθμός Μητρώου Ασφαλισμένου (Α.Μ.Α.) (A.M.A. - IKA book number)
• Αριθμός Μητρώου Κοινωνικής Ασφάλισης (Α.Μ.Κ.Α.) (A.M.K.A. - social insurance number)
• Αριθμός Φορολογικού Μητρώου (Α.Φ.Μ.) (A.F.M. - tax number)
In the last field, type the Κωδικός Οπτικής Επιβεβαίωσης (Code of visual confirmation) as shown in the frame just above it.
If you cannot read the Code, you can get a new one by clicking Ανανέωση (Refresh)
After completing the form, push the Είσοδος (Entry) Button.
On the next screen, push the printer symbol on the right hand side in the row where the Τύπος (Type) is Ιατροφαρμακευτική και Νοσοκ. Περίθαλψη (Medical and Hospital Care), to print the paper. Then fold it and keep it in your Personal Health Booklet. Source thecic eu
**UK Pensioners who intend to take up residence in Greece and who are entitled to free medical treatment in the United Kingdom or other EU country, also enjoy a similar entitlement in Greece. They should produce to their local IKA office form S1 (previously E121) which is issued by their local Department of Health Office in the United Kingdom, along with the required documents listed below and they will be issued with an IKA health book.
Documents required for IKA health book (Βιβλιάριο υγέιας) under the reciprocal agreement for form Form S1.
• form S1 ( old E121 & E119
• photocopy of passport
• you may also be asked to produce a residence certificate
• tax number 2 photographs
• An AMKA number
